SSS Retirement Form Download and Application Requirements

There are basically 3 types of SSS retirement forms that you can download from the official Social Security System website at sss.gov.ph. The thing is you should know first which amongst the 3 forms you need to fill up your personal information with so you can directly download it either from the SSS site or from here with our webpage at SSS Online Inquiry System.

SSS Retirement Form DDR2 Application Requirements

Death

Primary Beneficiaries

1. Death Certificate of member
2. Marriage Certificate
3. Birth/Baptismal Certificates of minors
4. Medical Certificate of incapacitated child, if any
5. Death Certificate of spouse, if already deceased
6. Application for Representative Payee (CLD-15)
7. Guaranteed Bond Form (BPN-107)
8. Proof of relationship such as record of birth, a statement before a court of record or any authentic writing/document

Secondary Beneficiaries

1. If Claimant is Parent

• Death Certificate of member
• Birth Certificate of deceased member
• Marriage Certificate of parents

1. If Claimant is other than Parents

• Death Certificate of parents
• Birth Certificate of the deceased brother/sister
• Birth Certificate of minor beneficiaries
